Our staff is available to schedule your appointment by phone Monday – Friday from 8:30am – 5:00pm at 313-821-3777.
When the office is closed there is always a physician on-call 24 hours a day 7 days a week at 313-821-3777.
Same Day Sick Appointments
If your child is sick feel free to call the office for a same day sick appointment and we will do our best to get them in.
What to Bring
For your appointment please bring the following items:
- Insurance Card & Picture ID
- Change of address and phone number (if applicable)
- Immunization Record
- Name of pharmacy, address and phone number
- Court appointed legal guardianship papers (if applicable)
- Any health forms or paperwork to be completed by the physician
- List of concerns about your child’s health
